- Please bring your Insurance Card and Photo ID such as a driver's license for accounting purposes.
- Please plan to arrive 10 minutes early to complete New Patient Forms.
- A short Office Tour and Pro-Adjuster Video will be presented to you prior to your exam with the doctor.
- Dr. Kloczkowski will conduct a brief review of your medical history, ask questions regarding your current condition and perform a thorough screening with the
Pro-Adjuster.
- Depending on your condition, other tests, x-rays, etc. can be performed or scheduled.
